Continuing Your Care

Can I continue seeing Dr. Najafi and his team?

Yes. Dr. Amir Najafi will begin seeing patients at Integra Heart in November 2026, supported by the Integra Heart team.

Where will Integra Heart be located?

Integra Heart will open at the same Bel Air office where Dr. Najafi previously cared for patients.

2021-B Emmorton Road, Suite 110
Bel Air, Maryland 21015

Appointments

What happens to appointments previously scheduled with Cardiovascular Associates of Maryland?

Appointments previously scheduled through Cardiovascular Associates of Maryland do not automatically transfer to Integra Heart.

To continue your care with Dr. Najafi, please submit a new appointment request with Integra Heart. For questions about an appointment at your former practice, contact that office directly.

When will Integra Heart begin seeing patients?

Integra Heart plans to begin seeing patients in November 2026 at the same Bel Air location.

You can submit an appointment request now. Our scheduling team will contact you to arrange and confirm a visit. Specific appointment dates and times are not guaranteed.

How do I request an appointment with Integra Heart?

Complete the short Request an Appointment form. Our scheduling team will contact you to arrange a visit as appointments become available.

You may also contact Integra Heart at (410) 314-3278 with general scheduling questions.

Is my appointment confirmed when I submit the form?

No. Submitting the form sends us an appointment request; it does not confirm an appointment.

Your appointment is confirmed only after the Integra Heart team contacts you and provides confirmation.

Insurance

Will Integra Heart accept my insurance?

Integra Heart participates in Original Medicare. Participation with other insurance plans, including Medicare Advantage plans, varies and may still be in progress.

Before your appointment, please contact Integra Heart at (410) 314-3278 to ask about your specific plan, and confirm your coverage and network benefits directly with your insurer.

Prescription Refills

How do I request prescription refills while Integra Heart prepares to open?

Until you have established care with Integra Heart, continue contacting the clinician or practice currently responsible for prescribing your medication.

Integra Heart is preparing to open in November 2026 and may not be able to process refills for patients who have not yet established care with the practice.

What should I tell my pharmacy about Integra Heart?

After you have established care with Integra Heart, ask your pharmacy to direct future refill requests to our practice.

Our clinical fax for pharmacies and healthcare professionals is (833) 455-9526. Requests sent to your former practice or health-system account will not automatically reach Integra Heart.

What if I need a refill before my first Integra Heart appointment?

Please contact the clinician or practice currently responsible for prescribing your medication. You may also ask your primary care clinician about a temporary refill, if medically appropriate.

Do not stop a prescribed heart medication without speaking with a healthcare professional. If you are running low on an essential medication, contact your prescribing clinician or primary care clinician promptly.

Phone & Fax Information

What are Integra Heart’s phone and fax numbers?

Use the following contact information for Integra Heart:

Office phone: (410) 314-3278
Clinical fax (pharmacies and healthcare providers): (833) 455-9526
General office fax (records authorizations and administrative correspondence): (410) 904-3278

Please use the clinical fax for referrals, pharmacy requests, medical records, and other clinical correspondence. The general office fax may be used for medical-records authorizations, administrative documents, and other general correspondence. Phone numbers, fax numbers, and patient portals from the previous practice do not route to Integra Heart.

Medical Records, Test Results & Messages

What will happen to my medical records, and how do I request them?

Your existing records remain with LifeBridge Health and do not automatically become part of Integra Heart’s medical records.

To authorize their release to Integra Heart, please visit our LifeBridge Records Request page for the paper authorization form and return instructions. Our Transition Steps page also provides online and paper options.

Requesting your medical records and requesting an Integra Heart appointment are separate steps. Submitting a records authorization does not mean that Integra Heart has already received your records.

What should I do about pending test results?

For testing ordered through Cardiovascular Associates of Maryland or LifeBridge Health, continue following up with the ordering practice for results and any necessary instructions.

Do not assume that a pending result, medical record, or message has automatically transferred to Integra Heart.

Can I continue sending messages through my previous patient portal?

No. Please do not use the previous LifeBridge Health or Cerner patient portal to contact Dr. Najafi or the Integra Heart team. Messages sent through that system do not reach Integra Heart.

For general questions, please call (410) 314-3278. Integra Heart will provide patient-portal instructions when you are enrolled with the practice.

Urgent Medical Needs

What should I do if I have an urgent medical concern?

For a medical emergency, call 911 or go to the nearest emergency department.

Do not use the online appointment-request form for urgent concerns, prescription emergencies, or time-sensitive medical questions.

For urgent, non-emergency medical needs before you have established care with Integra Heart, please contact the clinician currently responsible for your care or your primary care clinician.
