Online Records Authorization
Complete the LifeBridge Health online records request. Please follow our step-by-step guide as you complete the request so your records are directed to Integra Heart correctly.

Paper Authorization Form

A pre-filled form is available for patients who prefer a paper option. Please complete your information, sign the form, and return it to Integra Heart.

Step 2 — Request your records from LifeBridge Health

LifeBridge Health requires a separate authorization before it can release your existing records to Integra Heart. There are two options: Online or Print.
