Can I continue seeing Dr. Najafi and his team?
Yes. Dr. Amir Najafi will continue caring for patients at Integra Heart, supported by the Integra Heart team.

Appointments
What happens to appointments scheduled on or before September 30, 2026?
Appointments scheduled on or before September 30, 2026, will remain with Cardiovascular Associates of Maryland unless you are contacted and told otherwise.
What happens to appointments scheduled after September 30, 2026?
Appointments currently scheduled through Cardiovascular Associates of Maryland after September 30, 2026, will not automatically transfer to Integra Heart. We will make every reasonable effort to offer you an Integra Heart appointment as close as possible to your original appointment date and time. Please submit an appointment request so our team can coordinate your continued care. The exact date and time cannot be guaranteed, and your appointment will be confirmed after our scheduling team contacts you.
How do I request an appointment with Integra Heart?
Complete the short Request an Appointment form. Our scheduling team will contact you as appointments become available.
Is my appointment confirmed when I submit the form?
No. Submitting the form sends an appointment request but does not confirm an appointment.
Your appointment is confirmed only after you receive confirmation from the Integra Heart team.
Insurance
Will Integra Heart accept my insurance?
Integra Heart is working to participate with major insurance plans. Additional information will be provided as insurance participation is finalized.
Because coverage and network benefits vary, please also confirm participation directly with your insurance plan before your appointment.
Prescription Refills
What should I do about prescription refills before the transition?
Please review your cardiac medications now and request any needed refills from Cardiovascular Associates of Maryland before September 30, 2026.
When medically appropriate, obtain enough medication to avoid running out during the transition. There may be a brief period when Integra Heart cannot process refills for patients who have not yet been seen and established with the new practice.
What should I tell my pharmacy after September 30?
Until you have established care with Integra Heart, continue directing refill requests to the clinician or practice currently responsible for prescribing your medication. After you have established care with Integra Heart, please update your pharmacy with Integra Heart’s contact information. Refill requests sent through another practice or health-system account are not received by the Integra Heart team.
What if I need a refill before my first Integra Heart appointment?
Until you have established care with Integra Heart, please contact the clinician or practice currently responsible for prescribing your medication. You may also ask your primary care clinician whether they can provide a temporary refill, if medically appropriate. Do not stop a prescribed heart medication without speaking with a healthcare professional. If you are running low on an essential medication, contact the prescribing clinician or your primary care clinician promptly.

Which phone and fax numbers should I use during the transition?
For appointments and care through Cardiovascular Associates of Maryland on or before September 30, 2026, please continue using:

The date on which patients should begin using these numbers will be posted as Integra Heart's opening approaches.
During the interim, you may submit an appointment request through the Integra Heart website. After establishing care with Integra Heart, please update your pharmacy, primary care clinician, and other healthcare providers with our new contact information. Once you have established care with Integra Heart, messages sent to Cardiovascular Associates of Maryland or LifeBridge Health will not reach us. Please send all requests directly to Integra Heart.
Medical Records, Test Results and Messages
What will happen to my medical records, and how do I request them?
Your existing medical records will remain with LifeBridge Health and will not automatically become part of Integra Heart's system.
If you plan to continue your care with Dr. Najafi, LifeBridge Health requires you to complete a separate medical records authorization before it can release your records to Integra Heart.
Please visit our LifeBridge Records Request page for the LifeBridge request link and step-by-step instructions.
Requesting your records is separate from requesting an appointment, so please complete both. Completing the LifeBridge Health authorization does not necessarily mean that Integra Heart has already received your records.
What should I do about pending test results?
If you have testing performed through Cardiovascular Associates of Maryland or LifeBridge Health before September 30, please continue following up with the practice that ordered the test until you have received the result and any necessary instructions.
Do not assume that a pending result or message has automatically transferred to Integra Heart.
Can I continue sending messages through my current patient portal?
After September 30, please do not use the existing LifeBridge Health or Cerner patient portal to contact Dr. Najafi or the Integra Heart team.
Messages sent through the previous system will not reach Integra Heart. New contact and patient-portal instructions will be provided once you are enrolled with the practice.
Urgent Medical Needs
What should I do if I have an urgent medical concern?
For a medical emergency, call 911 or go to the nearest emergency department.
The online appointment-request form should not be used for urgent concerns, prescription emergencies or time-sensitive medical questions.
During the transition, please contact your current medical practice or primary care clinician for urgent, non-emergency needs.
